Grant Contract Forms and Information

Successful grantees are required to submit the following forms to RBAC with the grant application. These documents will become part of the grant project contract.

• No Overdue Taxes Certification

• Conflict of Interest Policy Template - place the text from this template on company letterhead and modify as needed. Or submit a copy of your organization's internal Conflict of Interest Policy

• Non-Debarment Form

• Parties applying for grants should consult Federal Circulars A-87 (PDF) and A-122 (PDF) regarding cost basis for proposed grant expenditures.
