Facility Forms

These forms are to be used for FY 17-18 Reporting by Solid Waste Facilities operating in North Carolina. When complete, a copy of the form(s) should be sent to your Regional Environmental Senior Specialist (Inspector), and a copy to the County Manager from which the facility received waste as required by law. Questions regarding the form should go to your inspector. If you are unsure who your inspector is, you will find their name on the form itself, after choosing the county where the facility is located. These forms are due to the inspectors no later than August 1, 2018.

Local Government Form

This form is to be used for FY 17-18 Reporting by all County and Municipal Governments in North Carolina (G.S. 130A 309.09A). Failure to report will result in the community being exempt from receiving disposal tax distributions in the future.

  • 1. The form is to be completed and returned no later than September 1, 2018.
  • 2. The annual report must be completed and submitted electronically. It is important to use "Save As..." when saving your work. We ask that you name the file with your community's name, for example: NashCounty_LGAR2018 or Burlington_LGAR2018. When complete, a copy of the form should be attached to an email and sent to the following email address:
