Funding Application Forms and Additional Resources
Funding application forms, guidance, and resource materials are available on this webpage for eligible entities interested in applying for funding for drinking water, wastewater, and/or stormwater projects. Applications are reviewed and presented to the State Water Infrastructure Authority for awards.

Rolling Applications. Rolling applications are accepted year-round. See the Applications Forms tab and Priority Rating Systems, Guidance Documents tabs below. Submit your completed Rolling application here. Rolling applications are available for the following funding programs only:
o Lead Service Line Replacement funding for drinking water systems (LSLR)
o Emerging Contaminants Evaluation/Assessment Study Projects for PFAS-affected drinking water, wastewater, and publicly-owned landfills (DWSRF-EC Studies and CWSRF-EC Studies)
o Decentralized wastewater treatment systems projects from the CWSRF Pilot Program (CWSRF DWTS Pilot Program)

Below are the forms and guidance on how to complete the application narrative to claim Priority Rating System (PRS) line item points. These are required as part of the application. Please select the appropriate one based on the type of application you are submitting.

CDBG-I REQUIRED SUBMISSION DOCUMENTS
CDBG-I Applications require both:
- a digital submission through the Division Submission Portal
- a mailed hardcopy original. Send the hardcopy via UPS/ Fedex to:
INSTRUCTIONS
Applicants must first download the core documents, the CDBG‑I Priority Rating System and Guidance & the Interactive Scorecard, from the Priority Rating Systems, Guidance Documents tab. These foundational documents explain scoring, required attachments, and how each tab must be assembled.
The additional materials listed below are supplements to those core documents and should be gathered and assembled in the order shown to ensure a complete submission.
FORMS FOR CDBG-I APPLICATIONS
Tabs 1–8 correspond directly with the “General CDBG‑I Assembly Checklist” found on page 21 of the CDBG‑I Program Priority Rating System and Guidance. Applicants should confirm they are using the latest version of each form and review the Priority Rating System (PRS) guidance pages cited in the checklist.
Please note: Required documentation must be submitted with the narrative sections to receive points. If any required documentation is missing, points will not be awarded. All required documentation is detailed in the Guidance document.

Rolling applications are only for
- lead service line identification and replacement projects,
- emerging contaminants (PFAS) evaluation/assessment study projects, and
- decentralized wastewater treatment system projects.
Rolling applications can be submitted on any day here. Rolling applications received by 5:00 pm on the following dates will be considered for funding at the next State Water Infrastructure Authority meeting as listed below:
