Individual Industrial Permits
Fee adjustments 2023
IMPORTANT UPDATE: Many permit fees have changed for applications received on or after October 3, 2023. North Carolina’s 2023 Appropriations Act Session Law 2023-134 includes fee adjustments for 28 Department of Environmental Quality (DEQ) programs, including Stormwater. A full list of fee adjustments is available on the DEQ website.
General permits apply to numerous broad categories of industrial activities with potential stormwater discharges. Industries that are not eligible for any of the general permits are required to seek an individual permit.
Apply for a New Individual Permit
Complete the following three steps:
- Complete both EPA Form 1 and EPA Form 2F.
- Mail one hard copy of the forms, supporting documents, and permit application fee to Brianna Young, 1612 MSC, Raleigh, NC 27699-1612 (Note: Do not mail the package to the EPA). (For a comprehensive list of program permit fees, see the DEMLR permit application fee table here)
- In addition, e-mail the forms and supporting documents to Brianna Young.
Renew an Existing Individual Permit
Complete the following three steps no later than 180 days prior to the expiration date:
- Complete EPA Form 1 and EPA Form 2F, the SPPP Certification Form, and the Supplemental Information Form.
- Mail one hard copy of the forms and supporting documents to Brianna Young, 1612 MSC, Raleigh, NC 27699-1612.
- In addition, e-mail the forms and supporting documents to Brianna Young.
Representative Outfall Status (ROS) Requests
To request or continue ROS designation for one or more stormwater outfalls, complete these steps:
- Initial ROS Request: Submit the completed Representative Outfall Status (ROS) Request Form to the Stormwater Upload Form. Mail one hard copy of the form and supporting documents directly to the appropriate Regional Office. There is no fee for a representative outfall status request.
- Continue ROS into a new permit term: If you had Representative Outfall Status in the previous permit term, use the the Stormwater Upload Form to submit both (1) the completed ROS Renewal Certification Form and (2) prior documentation that was provided when the Division originally granted ROS.

How to Submit Paper DMRs Before eDMR is Implemented
The Stormwater Program is planning for NPDES Stormwater Industrial individual permitted facilities to use electronic Discharge Monitoring Reports in 2022. Until your facility begins using eDMR, follow this NEW procedure for submitting eDMRs:
- Download the Generic Individual Permit DMR Form.
- Complete, sign, scan and upload the DMR to the NPDES Stormwater Permit Discharge Monitoring Report (DMR) Upload form within 30 days of receiving sampling results.
- Mail the original, signed hard copy of the DMR to the appropriate DEMLR Regional Office.
- Do not send DMRs to Central Files in Raleigh! (This is a change from the previous procedure.)
- Note that DMR forms have recently been updated.
